Client Privacy Policy Statement

Shoalhaven Community Transport Service Inc. may release information about Home and Community Care Clients (without identifying them by full name or address) to the NSW Department of Ageing Disability and Home Care and the National or State Data Repository.

This will enable the collection of information about Home and Community Care Client services and their consumers. The information will be kept confidential. This information is to be used for statistical purposes only and will not be used to affect client entitlements and or their access to services. This statement concerns only the Home and Community Care Minimum Data Set collected and transmitted to the National or State Data Repository. This data is sent in a code format.
