How can people access our service?
Clients can be referred by doctor, family, friends, neighbours, nurses, social workers or themselves.
The assessment process.
All assessments are done over the phone by one of the administrative staff, who ask a series of questions from a “Client Information and Referral Record” known as a CIARR form, this information then identifies whether the person is eligible for the service or not. If the person is ineligible the staff try and offer alternative suggestions for transport.
The types of transport offered
We provide transport for individual medical appointments, group transport for shopping, social recreation, day respite and weekend outings. Priority is given to Medical and Hospital transport. Sometimes transport cannot be provide due to time constraints and vehicle availability.
INDIVIDUAL SYDNEY TRIPS BY CAR
Unfortunately we are unable to return people who have had procedures on the day from Sydney, but it may be possible to transport these people the following day.
Bookings are essential with at least three days prior notice. Sometimes transport cannot be provide due to time constraints and vehicle availability.
INDIVIDUAL WOLLONGONG TRIPS BY CAR
The first appointment time in Wollongong is 9.30am with the latest appointment being 2.30pm.
Unfortunately we are unable to return people who have had procedures on the day but it may be possible for us to organise a nursing service for the return trip at a small cost.
